It's separate. The Bali Tourism Levy (Pungutan Wisatawan Asing / 'Love Bali' tax) of IDR 150,000 (about USD 10) per person is a Bali provincial fee, unrelated to your visa fee, and it isn't included in the visa prices.
You can pay it yourself in advance at lovebali.baliprov.go.id or on arrival at the airport — it only applies if you're visiting Bali specifically.
